Advanced Healthcare Administration (AHA) is a CalAIM Community Support Services Provider, working with the California Department of Healthcare Services since 2014.
Social determinants of health are the conditions in which people are born, grow, live, work, and age. These factors significantly impact individual and population health outcomes.
At AHA, we understand the critical role that social determinants of health play in overall well-being.
